Student Counselling Request
If you would like your child to see the School Psychologist please fill out the following form.
Student Leave Form
If your child requires more than 3 days of leave from College please fill in this form.
To be used to apply for leave for Medical/Illness, Sport Representation, Travel, Family Emergencies, etc
Please note you will have to supply relevant documentation eg. itinerary, medical certificate.
Assessment Extension Application (Years 7-10)
A parent or doctor's letter must be attached to this application.
This Form is to be Submitted to the Faculty Coordinator.
Assessment Extension Application (Years 11-12)
A parent or doctor's letter must be attached to this application
This form is to be submitted to the Academic Dean of Students.
Assessment Task - Misadventure/Illness Appeal (Years 11-12)
A separate Illness / Misadventure appeal form must be completed for EVERY missed task and must be received within 3 school days after the due date of the Assessment Task / Examination.
This form is to be submitted to the Academic Dean of Students.
